The heart behind our name
Jazzy Hearts Foundation was founded by Teresa and Jackie Carter to honor the legacy of their daughter, Jazlene “Jazzy” Carter, a heart transplant recipient whose life was a testament to resilience, faith and God’s glory.
Diagnosed with restrictive cardiomyopathy at just 10 months old, Jazzy faced rare diagnoses, multiple near-death experiences and extraordinary medical interventions throughout her journey.
Although she tragically passed away due to cardiac allograft vasculopathy following her heart transplant, her courage and spirit continue to inspire others.
Today, Jazzy Hearts Foundation carries her legacy forward by raising awareness of congenital heart disease and organ donation, supporting research to improve the longevity and well-being of pediatric heart transplant recipients, and offering hope to families navigating similar journeys. Through Jazzy’s story, the organization strives to turn one child’s resilient spirit into hope, healing and lasting impact for others.

Founded by Teresa & Jackie Carter, in loving memory of Jazlene “Jazzy” Carter
